At HDC, we know that staying healthy, independent, and connected can take more than one type of support. That’s why we offer a variety of programs designed to help older adults continue living safely, confidently, and with dignity in their own communities.
Services vary by county and may include:
Home Delivered Meals
Nutritious meals delivered to eligible seniors in Sanilac and Tuscola Counties.
Transportation Assistance
Help with transportation needs for eligible seniors in Sanilac and Tuscola Counties.
Elder Advocacy
Support for older adults experiencing abuse, neglect, exploitation, or financial harm in Sanilac, Tuscola, Lapeer, and Huron Counties.
Incontinence Supplies
Protective undergarments provided at no cost for eligible individuals in Tuscola, Sanilac, Lapeer, and Huron Counties.
Family Caregiver Support
Resources, education, and support for caregivers in Tuscola and Sanilac Counties.
Case Management Support
One-on-one guidance, education, resources, and tools to help older adults age safely at home in Tuscola and Sanilac Counties.
Senior Outreach
Connection to resources, answers to questions, and guidance for older adults and families in Tuscola, Sanilac, and Huron Counties.
Community Activity & Dining Centers
Welcoming spaces for meals, activities, and connection in Sanilac and Tuscola Counties.


At our Senior Centers, we provide more than just a meal; we well-balanced, nutritious food in a group setting where friends gather and new connections are made.
We care about your health, your happiness, and your sense of belonging. That is why our centers also offer opportunities to take part in activities that bring people together, encourage laughter, and keep both mind and body active. If you are 60 or older, we would love for you to join us!

Our HDM program provides balanced meals delivered to homes of seniors who are unable to shop or prepare food for themselves. More than just a meal, this service also offers a friendly check-in from a caring driver, helping seniors maintain independence while knowing someone is looking out for their well-being.
Eligibility is generally for individuals age 60 and older who are homebound or have difficulty preparing meals. Available to Tuscola and Sanilac County Residents. Temporary delivery may be provided after a surgery or an accident as well.
